To fill out this application you will need the following:

  • Social Security Number
  • Home address history for the past five years
  • Current driver’s license number and driver’s license history for the past 5 years
  • Employment History last 10 years
  • History of traffic accidents, violations, and/or convictions for the past 5 years (including DUI or reckless driving convictions and suspensions)
  • Criminal History
  • Military History (if applicable)

To qualify with BDS, Inc. you must meet the following criteria:

  • Must be at least 23 years of age.
  • Must have a least 2 years of verifiable, recent OTR experience
  • Must have no more than 3 moving violations in the past three years; no more than 2 in the past year.
  • Must not have any major violations (DWI, DUI, Hit/Run, Reckless Operation) with suspension in the past 5 years.
  • Must not have any positive drug or alcohol test results in the past 5 years.
  • No more than 2 preventable DOT recordable accidents in the last 3 years.
